Register My Athlete
All athletes must create an account at Register My Athlete. Once you create an account you will find all the physical forms needed. Be sure to complete all items in order to be eligible for your sport. You will receive an email when you are cleared for tryouts. Create your Register My Athlete account here.
Brainbook Concussion Course
All athletes are required to complete the Brainbook Concussion course and upload the certificate of completion to your Register My Athlete account. You can find that course here. Register as a Boulder Creek Student.
Dr. Physical Forms
These forms must be completed by your child's doctor after their physical examination. Once completed, please upload to the RMA site.
