Required Documentation
Important! In order to participate in any DVUSD athletic activity, the following information must be completed and turned in to the school's athletic department via Register My Athlete:
1. A certificate of completion for the online courses "Opioid Education: The Game You Can't Win" and "Brainbook" must be on file at the school. These courses must be completed prior to any participation, and need to be completed only once during the school year by the student-athlete.
2. A copy of the participant's birth certificate needs to be on file at the school.
3. Your child's annual physical evaluation. Parents must use the AIA Student Physical Forms for their student-athlete's physical. Physicals received after March 1, 2025, are acceptable for the 2025-26 school year. Please download, print, and take the physical evaluation form to your child's doctor to fill out.
4. Complete all other forms on the Register My Athlete website.
Student-Athlete Education
A certificate of completion for the online courses "Opioid Education: The Game You Can't Win" and "Brainbook" must be on file at the school. These courses must be completed prior to any participation, and need to be completed only once during the school year by the student-athlete.
- Access the AZ Preps 365 Education website.
- Register as a student, or login with your previous credentials.
- Take the tests identified with your name (not as a guest).
- Fill out the information page, and select all sports you plan on trying out for this school year.
- Middle School Students: When selecting the AIA Member School, select the high school you will likely attend. You will also select grade 9, regardless of which grade you are currently in.
- Complete the courses, and don't skip any sections. Brainbook takes approximately 30 minutes to complete, while Opioid Education will take approximately 20 minutes to complete.
- After completing the courses, save your copy of the completion certificates. You will later upload these to Register My Athlete.
7th & 8th Grade Eligibility
In accordance with the State of Arizona’s regulations and Deer Valley’s Governing Board policy, only those students deemed academically eligible may participate in extra or co-curricular activities.
Athletic eligibility will be determined every week by the school's administration and the athletic coordinator starting the first Monday after the week of tryouts.
- If a student-athlete is failing a class (a minimum of one "F"), they will be put on a pending academic list for one week. During this pending period, the student-athlete will still be considered on the team and allowed to participate.
- If a pending student-athlete is still failing a class (a minimum of one "F") the following week, the student-athlete will be put on an ineligible list for one week. During this ineligible period, the student-athlete will still be considered on the team and allowed to practice, but not allowed to participate in games or travel with the team to away contests.
- If the student-athlete has a combination of three occurrences during a season (pending or ineligible), then they will be dismissed from the team. This will be considered the "three strikes" rule.
- If a student-athlete is put on the academic pending list, a letter will be sent home with the student-athlete. If a student-athlete reaches the level of strike two (a combination of pending or ineligible), the coach should make direct contact with the parent/guardian to inform them of their student’s academic eligibility status.
- Grade checks will be run by the administration and/or the athletic coordinator by using PowerSchool every Monday morning, and the pending/ineligible time period will be Monday-Saturday. Grades are considered for all classes with no exceptions, and they are not looked at on a class-to-class basis. In the case of a teacher error, then that situation will be dealt with on an individual basis.
- Students assigned to OCR/ISS or students who are suspended from school may not participate in any team practices/games from the time of their assignment to OCR/ISS or home suspension until the day following their last day of assignment to OCR/ISS or home suspension. It is the responsibility of the coach to assure that no students assigned to on-campus suspension or home suspension are allowed to participate.
- Students and parents should be aware that if the student is medically excused from participating in regular physical education classes, they could be ineligible for participation in school athletic programs as stated on the medical excuse form by the parent or the student's doctor.
- All students are eligible on the 1st Monday after the last day of the quarter. For the 2025-26 school year, these dates are: October 13, January 12, and March 23.
Grade checks will not be concluded until the 3rd week of the new quarter. For the 2025-26 school year, these dates are the weeks of: August 18, October 27, January 26, and April 6.

Athletic Fees
ATTENTION
Athletics participation fees have increased for the 2025-26 school year for middle school and high school athletes, as well as high school marching bands and AFJROTC color guard units. See the new fees listed below.
Fees were increased in part because of the failure of the M&O override passage in November 2024. The increased rates were approved unanimously (5-0) by the DVSUD Governing Board on May 13, 2025.

ATHLETIC FEE PAYMENTS
- It is the parent’s or guardian's responsibility to keep receipts when paying for multiple family members in regards to tracking the family maximum amount being met.
- Athletic fees are non-refundable.
- The DVUSD Athletic Fees do not guarantee playing time in competition.
- Financial hardship waiver forms are available, email Grace Christiansen for more information.

Girls Volleyball tryouts will be held November 12th & 13th from 6:15 a.m. - 7:45 a.m. The girls will have access to the locker to get ready for the school day after tryouts.
Boys Volleyball tryouts will be held November 10th, 12th & 13th afterschool from 3:15 p.m. to 4:45 p.m.
